To register for a CNA Training, there’s a form that you’ll need to fill out. Don’t worry – we’ve created a handy checklist to help you keep track of it.

Abilities For a Future CNA & Equal Opportunity Statement – SIGNED
Neway Directions CNA Application & Tuition Agreement – FILLED OUT & SIGNED
State Required Background Disclosure – FILLED OUT & UPLOADED
Rules of Neway Directions Nurse Aide Training – SIGNED
2-Step TB Test Form – (2 separate skin tests (4 trips to the clinic) at least a week apart, but within 21 days of each other). It is the student’s responsibility to have this done and documentation turned in to Neway Directions. More information is in Step 5. – UPLOADED
COVID Vaccine Certificate – UPLOADED
Proof of Flu Vaccination (September – April) – UPLOADED
